Patient Management

  • Daily Engagement: Meet with patients first thing in the morning to discuss their progress and needs.
  • Detailed Oversight: Maintain an in-depth understanding of each patient's medical, psychological, and emotional status.

Reporting

  • IPD Rounds: Prepare and present detailed reports during In-Patient Department (IPD) rounds.
  • Decision Documentation: Ensure all decisions and actions are thoroughly documented and communicated to the Senior Resident.

Diagnosis and Treatment

  • Accurate Diagnosis: Strive for precise and timely diagnosis in collaboration with the healthcare team.
  • Psychoeducation: Provide patients with necessary psychoeducation to aid their understanding and recovery.

Planning and Counseling

  • Discharge Planning: Develop and implement comprehensive discharge plans tailored to each patient’s needs.
  • Counseling Sessions: Conduct individual and family counseling sessions to support patients and their families.

Coordination and Communication

  • Bridge Role: Act as the central coordinator between different aspects of patient care, including medical staff, patients, and their families.
  • Linking Care: Ensure seamless communication and integration of care components to facilitate effective recovery.

Qualifications

  • Education: Bachelor’s/Master’s in Clinical Psychology.
  • Experience: Freshers/Minimum 1.5 years of experience as a Case Manager.

Work Details

  • Mode: Full-time, on-site role (Work From Office - WFO).
  • Location: Gurgaon, Haryana.
  • Working Hours: 9 hours a day, 6 days a week, with a rotational week off.
